CHIP - Highmark Healthy Kids Special Needs and Case Management

What is it?

The CHIP - Highmark Healthy Kids Special Needs and Case Management program helps children with serious medical conditions and special health care needs get the care they require — offering free family-centered, community-based care coordination that works to improve the quality of life for children and their parents enrolled in Highmark Healthy Kids.

What does it cover?

Care Coordination

If your enrolled child has a special health condition, our Highmark Case Management team can review your child's health needs to decide how to best support you and your child. We can explain how to manage your child's health, the "what" and "why" of medications, testing, and overall wellness. We can also suggest ways to make getting care easier and share health information and other resources.

Case and Disease Management

Conditions eligible for disease management include but are not limited to, Asthma, Diabetes, Obesity (Healthy Habits), and Tobacco/Vaping.  You can download the pdf’s to learn more about each condition.

You can also learn about ways to build healthy habits with your children in our Family Wellness brochure including healthy eating ideas, exercises for the whole family, improving mental health, and making time for family fun.

How much does it cost?

The Special Needs and Case Management Program is available free of charge to children enrolled in Highmark Healthy Kids, a CHIP HMO.

Families' Highmark Healthy Kids benefits or eligibility are NOT impacted by participation in the Special Needs and Case Management Program.

Am I eligible?

You're eligible if your child is enrolled in CHIP and you live in Highmark's service area.

Why should I use it?

The program can benefit children who:

  • Use the health care system much more than their peers.
  • Miss a lot of school due to illness.
  • Have special health care needs that caused parents to cut back or stop working.
  • Require specialized services and therapies, home health services, durable medical equipment, family therapy, early intervention services, special education, or social services.
  • Have severe persistent asthma, type 1 diabetes, obesity, or those who use tobacco or vaping products.

Early intervention tools 

Children between the ages of 0-5 who have special needs due to a developmental delay or diagnosis are eligible to receive this type of care.

The CONNECT Helpline assists families in locating resources and providing information regarding child development for children ages birth to age 5. CONNECT can assist parents by making a direct link to their county early intervention program or local preschool early intervention program. To make a referral for early intervention, please call the CONNECT Helpline at 1-800-692-7288.
