SDoH: Support for mom and baby
See how SDoH barriers were removed and one member was given the support and resources she needed to navigate a difficult pregnancy. With the help of our social workers and case managers, we are able to help our members overcome SDOH related challenges and feel their best.
SDoH include the social, physical and economic conditions in which we live our daily lives. They affect our whole health, including mental health. The following 5 factors are considered especially important determinants of health:
Economic stability: Financial resources, employment, food security, and housing stability.
Education access and quality: Early childhood development, language, literacy, high school graduation, and access to higher education.
Health care access and quality: Access to and understanding of health services and own personal health.
Neighborhood and built environments: Quality of housing, access to transportation, availability of healthy foods, and neighborhood crime and violence.
Social and community context: Social cohesion, levels of discrimination, and workplace conditions.
