    Well in Place

    Well in Place is a service that Highmark offers free of charge to members who would benefit from specialized care coordination and/or supportive care while maintaining their independence in the community.  

    What to expect from the Well in Place team

    An interdisciplinary care team led by a specialist physician and including an Advanced Practice Provider (APP; nurse practitioner or physician assistant), Registered Nurse (RN), Licensed Social Worker (LSW) and Care Coordinator.  Our team of clinicians are ready to help support you with: 

    • Working towards your goals: We will work with you and your family to help ensure your care plan aligns with your health care wishes.
    • Coordinating your care: Our team works closely with your doctors and health care providers. We can help identify additional resources that may be beneficial to you.
    • Managing your symptoms: Our team helps you manage symptoms such as pain, shortness of breath, fatigue, constipation, nausea, loss of appetite, difficulty sleeping, and depression.

    Educating you and your loved ones: We will make sure you, and your family, understand your illness, care plan, and treatments. We want you to feel confident in your health care decision-making and your future plans.
