Applying for Medicaid is easy.

We’ll help you learn how to get yourself and your family covered.

Qualifying for Medicaid

Before you can apply, you’ll need to know if you qualify for Medicaid. To begin, you’ll answer a few questions on the WV Path website.

Now that you’ve qualified, you’ll have four ways to apply for Medicaid.

 

Phone

Call Mountain Health Trust at 1-800-449-8466 (TTY: 711 or 1-304-344-0015).

U.S. Mail

Print and complete your application before mailing it to your county office.

You’ll need to visit your local DoHS office to apply in person.

Choose your health plan

You’ll get a welcome packet in the mail when your application is approved. You can select Highmark Health Options as your Medicaid plan at this time.

Note, if you don’t choose a plan, the state will choose one for you. Also, if you’re already enrolled in a healthplan, you can change at any time.

Extra benefits 

Some of the benefits that Highmark Health Options covers are “extra” benefits. This means they aren’t the standard benefits that all Medicaid plans cover. As examples, our members can get:

  • An extra $500 toward dental care (for members age 21 and older)
  • Rewards for by completing healthy activities like having wellness visits and screenings
  • Help with preventing diabetes (for eligible members age 18 and older)
  • Rewards for going to prenatal care visits and helpful (expecting and new moms)
  • Helpful pregnancy apps
