Actions to report

There are many kinds of wrong actions that harm both adults and children. These actions are critical incidents.

Denying basic needs

Basic needs are the things we need to live day-to-day, like:

  • Food
  • Medicine
  • Personal care items

Physical harm

Physical harm may include:

  • Causing a severe injury, like a broken bone, deep cut or serious burn
  • Forcing or tricking a member into sexual activity
  • Leaving a member unsupervised

Mental harm

Actions like these may cause mental harm:

  • Injuring a member
  • Causing emotional distress
  • Yelling at a member

Financial harm

Financial harm may be caused by:

  • Stealing a member’s money or medicine
  • Using a member’s checkbook, debit card or credit card without permission
  • Refusing to do the job

As a reminder, these are examples. If something happens that doesn’t seem right, it could be a critical incident. Wrong actions are critical incidents.
