Here’s what you can start doing now and some of the ways we’ll communicate with you before your D-SNP coverage begins. 

Get an early start

You can begin using our tools for setting up your new D-SNP benefits as soon as you apply. Not waiting means you can use your benefits as soon as you need them.

Your D-SNP communications

These are some of the communications you’ll receive before your coverage begins. 

Member Access Card with Confirmation Letter  

When you’re approved, you’ll get a confirmation letter with your Member Access card via U.S. Mail. This is the card you’ll need whenever you get care or a prescription filled.  

Member Packet

You can review D-SNP benefits documents and helpful information when you receive your Member Packet. 

Welcome phone call 

Our Member Services Team will call to welcome you and talk about scheduling a Health Risk Assessment (HRA) with your Primary Care Physician (PCP). Your HRA helps identify any areas you may need to work on to stay healthy.

Highmark Health Options Duals is offered by Highmark Blue Cross Blue Shield. Highmark BCBSD Inc. d/b/a Highmark Blue
Cross Blue Shield offers HMO plans with a Medicare Contract. Enrollment in these plans depends on contract renewal.
