Requesting new materials

We want to help our members understand their forms, Member Handbook and newsletters.  

If you need or prefer different benefits materials, you have options. To make a request, please call us at 1-833-957-0020 (TTY: 711) any time between Monday - Friday, 8 a.m.– 5 p.m. We'll send you new materials at no charge.  

Forms

We can send you member forms in many ways, including:  .

  • Large print  
  • Audio  
  • Digital 
  • Braille  

We can also provide forms in a different language.

Different versions of these materials are available.
