Healthy Transitions Meal Delivery

You're covered to receive free meal delivery to your home if you've been discharged from: 

• An inpatient hospital 
• A rehab facility 
• A skilled nursing facility 

Once discharged from the facility, a case manager will reach out to you to coordinate the meal plan and delivery. This benefit covers up to 14 meals over seven days.  

Hearing

Highmark Health Options D-SNP hearing benefits are provided by TruHearing. As a member, you’ll get a Medicare-covered hearing exam with your D-SNP.  Extra plan benefits also cover:  

• Routine hearing exam - 1 exam each year at $0 copay 
• Routine hearing aids - 2 hearing aids (1 per ear) every 3 years

For hearing aids, you’ll need to call to make an appointment with a participating TruHearing audiologist.

SSBCI benefits 

Depending on your eligibility, your Highmark Health Options Duals plan includes monthly benefits to help you stay healthy. 

• If you qualify for Special Supplemental Benefits for Chronically Ill* (SSBCI): You'll receive $175 each month. You can use this on things like over-the-counter medicines, home & bathroom safety items, healthy food, utilities, gas at the pump, and everyday essentials. Be sure to use your funds by the end of the month because any money you don't use won't roll over.
